Applegate HomeCare & Hospice is a local, family-owned Utah company that has been providing high-quality health care services to our community for over 30 years. We have seven branches servicing the Wasatch Front, Summit County, Box Elder County, Davis County, Wasatch County, Morgan County, and Southern Utah.
Our Management Team
Wes Hansen, CEO
Wes has been in health care administration for over 25 years. He has overseen skilled nursing facilities, assisted living facilities, and independent living facilities. These roles have allowed him to become expert in various areas such as human resources, hospitality, physical facilities and fiscal management. His infectious enthusiasm, personable nature and passion for his work have inspired many of his employees to call him one of their greatest leaders. Wes received a Bachelor’s Degree in Finance with a minor in Economics and Korean from Utah State University. In his spare time he loves to learn, laugh, hike, and read. Mink Creek, Idaho is his center of his universe; when he and his wife Heather are there, things stand still.
Lisa Hernandez, RN Vice President of Clinical Services
Lisa has been a caregiver most of her life. She started working with individuals with disabilities at a very young age, then transitioned from a CNA, to an LPN, and then an RN at Ogden Regional Medical Center for many years. She found her love and passion in HomeCare and Hospice 13 years ago and has been embedded ever since. She is very passionate about serving others and enhancing the lives of others! Lisa studied at Weber State University and Western Governors where she achieved two degrees: one in Health Science and the other in Nursing. She speaks fluent Spanish and is married with two beautiful children. She loves the outdoors, sports, her family, and traveling.
Mark Singerhoff, VP of Information Systems
Mark has been in the healthcare administration field for over 25 years. He has spent the last 19 years in the HomeCare and Hospice arena. He oversees the Intake department and the Volunteer Services department for Applegate HomeCare & Hospice. Mark was born in New York and grew up in the rural hills of Upstate Pennsylvania. He attended Weber State where he received an Associate Degree in Art and Human Services. Golf is his “sport” of choice.
Don Woodbury, VP Marketing
Don has spent thirty years in healthcare management in a variety of settings, with the last twelve years being in home health and hospice. He oversees business development and marketing efforts. Don is passionate about quality improvement and working to create delighted customers. He received a Master of Health Administration from Brigham Young University, completed majority of coursework for a Master Degree in Counseling from the University of Utah, and received a Bachelor’s Degree in Special Education and Psychology from Utah State University. For the most part, Don was raised in small Utah and Iowa towns and still loves the outdoors and open space. Music is his escape.
